Client intake form
The intake form doubles as an expression of interest. Complete it if you would like to register interest, and complete it before your consultation if you have already booked one — it means we can spend the whole fifteen minutes on your student instead of on details.
It covers your student's year level, the challenges you are noticing, what you are hoping to change, and your preferred session times.
How your information is used. Anything you submit is used only to prepare for your consultation and to shape sessions around your student. It is not sold and not shared for marketing. Full detail is in our privacy notice.

Pricing & cancellation
$110 per hour, $900 for a ten-session term package, $500 per week for intensives — plus the full cancellation policy.
